Refraction$30.00The evaluation of a patient’s vision, usually incorporated as part of a Routine Eye Health Exam. This service may on occasion be performed as a separate entity but only when an individual’s situation warrants and at the discretion of the examining doctor. The patient is provided with a copy of the eyeglass prescription. Routine Eye Health & Vision Exam- Established Patient -$122.00A thorough health evaluation of the eye as well as a refractive examination of the vision. The patient is provided with a copy of the eyeglass prescription. - Non Established -$128.00A thorough health evaluation of the eye as well as a refractive examination of the vision, but with the additional protocol of creating a new patient file and establishing patient baselines. The patient is provided with a copy of the eyeglass prescription. Routine Eye Health & Vision Examwith a Single Vision Contact Lens Evaluation- Established Patient -$193.00- Non-Established -$199.00In addition to a thorough evaluation of the eye’s health and the refractive examination of the vision, the patient is also evaluated to be fit with contact lenses. Because of how contacts rest directly on the surface of the eye, additional evaluations are required to both maximize comfort as well as avoid any negative effects on the health of the eyes. Often the corrective powers required to obtain good vision with contact lenses are different from that of the eyeglass prescription. Other parameters which must be considered for optimal results are the determination of the most appropriate lens material, mapping the eye’s curves in order to match it with the corresponding curves of the contact lens, and finally, deciphering the astigmatism axis, if needed. Typically, as parameter adjustments are warranted, there will be a trial evaluation period, during which the patient is provided with a set of contact lenses to wear for a prescribed period of time (such as a week) after which the patient returns to allow the doctor and staff to evaluate the corneal health effects of wearing contacts lenses. With the exception of “gas permeable” hard contacts, the trial lens and the follow up exam/exams is/are included in the original fee. In the past, patients have volunteered that no changes in their contact lens prescription are needed. In order to provide the best and proper medical care, as well as to stay compliant to the rules mandated by insurance regulations and the medical boards, Bowen Eye Care can no longer assume that the patient’s perception that "no changes are needed" is correct. The industry standard of performing a contact lens exambefore releasing a contact lens prescription will be followed. Extra testing for a contact lens evaluation which is not included in a routine eye health & vision exam includes keratometry, topography, and tear film/corneal evaluation for toxicity related to solutions and contact lens materials. Routine Eye Health & Vision Examwith a Multi-Focal/Bifocal/Rigid Gas Permeable Contact Lens Evaluation-Established Patient-$211.00-Non-Established-$217.00Exactly the same as the Health & Vision evaluations made in order to be able to wear single vision contact lens, but now adding another level of fitting parameters to satisfy the presbyopic or astigmatic patient. When evaluating contacts with both far and near focal points, typically, additional exam time as well as additional follow up visits are required. Routine Eye Health & Vision Examwith a Post LASIK /Keratoconus /RK Contact lens Patient Evaluation-Established Patient-$302.00-Non-Established-$308.00Similar to the previous contact lens exams but with a patient who has previous history of refractive surgery (LASIK, PRK, RK, etc.) or has Keratoconus, all which make the fitting of contacts on the surface of the eye much more difficult. Medical Eye Exam$66.00 - $302.00This is the examination and treatment for medical conditions such as abrasions, contusions, foreign bodies, and other eye traumas, as well as infections and diseases such as conjunctivitis, glaucoma and the effects of diabetes on the health of the eye. This fee is usually covered by most medical insurances with the patient only being responsible for their medical co-pays. The patient may receive a copy of their eyeglass prescription if a refractive examination of the vision is deemed to be necessary to evaluate the medical condition. Post Lasik Co-Management$530.00After being referred to this office by the examining surgeon, Bowen Eye Care will oversee the post-surgery recovery process. This is a global fee that will cover all necessary visits related to the surgery for a period of one year. Retinal Photos$69.00Used when medically advisable, these are photos of the retina or the back of the eye, and are used to photographically establish a baseline in order to chart eye changes or the progression of injuries or diseases. Comprehensive Glaucoma Work Up$289.00Used as medically advisable whenever the patient is either a glaucoma suspect or has been previously diagnosed. RGP Modification - per lens$25.00An in-house modification of a RGP contact lens that results in an actual change of the prescription or a physical fit of the contact lens. RGP Polish - per lens$25.00Removes protein build up and restores clarity in a RGP contact lens. Mainly recommended for RGP's less than 2 years old. Pupil Dilation-No Charge during the course of a routine eye examination-Bundled into the cost of a Routine Eye Health Exam, this service aids diagnosis by allowing the doctor greater access in viewing the interior of the eye. Dilation duration will vary from patient to patient but generally wears off within 4 to 8 hours. Most patients report that their distance vision is unaffected but their near vision is blurry temporarily. Most patients are sensitive to sunlight for a few hours. You may either wear your own sunglasses or temporary sunshades will be provided. OptoMap$36.00Used often in place of pupil dilation, the patient has the option of having special, low light photographs taken of the back of the eye without eye drops. The photos are saved in the patient’s permanent medical record and therefore can be reviewed at a later date for comparative purposes. Eyeglass Adjustments-No charge-Bowen Eye Care is happy to provide this service to our patients and customers whether they have purchased glasses from us or not. In addition to this service we are also happy to provide minor eyeglass repair services such nose-pad replacements and lost screw replacements. While our opticians and optical technicians are trained and experienced in the art of such adjustments and minor repairs, we can accept no responsibility for broken frames or scratched or chipped lenses or any other damages that may occur as a result of the performance of these services. |
